For your convenience and privacy, you may complete your initial paperwork at home prior to your appointment at Urologic Associates of Montgomery County. This is intended to make your visit more efficient and less cumbersome upon arrival. We request that each patient complete a Patient Demographic Sheet and a HIPPA form.
